Please ask a parent or guardian to complete the Medical Consent form for each player.

We recommend that you  photocopy a set of the completed consent forms, put them in a gallon ziplock bag, and ask your team's coach to keep them in his coaching bag for all practices & games.  Please keep the originals in your team binder.

The "Participant Code of Conduct" should be completed by Players, Coaches, & Managers for all Mite through Midget teams.  Please keep the originals in your team binder.
